Clarification Text Purpose and Scope

As Papatya Oral and Dental Health Services Trade Limited Company, we take measures to protect your personal data within the scope of the Law on Protection of Personal Data No. 6698 (referred to as "KVKK" or "Law"). We process your Personal Data within the scope of KVKK and relevant legal legislation, and in our capacity as "data controller", for the reasons and methods described below.

Papatya Oral and Dental Health Services Trade Limited Company Information on the Processing of Personal Data, pursuant to the article titled “Informing Obligation of the Data Controller” in Article 10 of the KVKK: the identity of the data controller, the method and legal reason for collecting your personal data, the purpose for which this data will be processed, to whom and for which purpose. It has been prepared for the purpose of informing you in the most transparent way about the data processing period and your rights listed in Article 11 of the KVKK. The explanations made for your "Personal Data" in the clarification text also include your "Special Quality Personal Data".

Collection Method and Legal Reason of Your Personal Data

Your personal data may be collected verbally, in writing or electronically, by means of automatic or non-automatic methods, by the affiliated units of the company, the website, social media channels, call center, mobile applications, software used to carry out activities within the company, by means of camera footage. Your personal data can be created and updated as long as your relationship with the Company continues, and can be processed and kept in both digital and physical environment.

Your personal data, in line with the purposes set out in the title of "Your Personal Data Processed and the Purposes of Processing" and in accordance with the provision of express consent within the scope of Article 5, paragraph 1 of the Law No. 6698, or as specified in the 2nd paragraph of Article 5;

a) It is clearly stipulated in the laws,

b) It is compulsory for the protection of the life or physical integrity of the person or another person, who is unable to express his consent due to actual impossibility or whose consent is not given legal validity,

c) It is necessary to process the personal data of the parties to the contract, provided that it is directly related to the establishment or performance of a contract.

ç) It is mandatory for the data controller to fulfill its legal obligation,

e) Data processing is mandatory for the establishment, exercise or protection of a right,

f) Data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject,

Your special quality personal data, which is processed as "association membership, union membership" outside of health and sexual life, can be used in accordance with "Your Personal Data Processed and Processing Purposes" in order to be used in case you want to benefit from special discounts for associations and unions with which our company has a contract, in accordance with Article 6 of Law No. 6698. It will be processed in cases stipulated by law in accordance with the provision of express consent within the scope of paragraph 2 of article 6 or paragraph 3 of article 6.

Article 6 of the Law No. 6698 in order to determine your preferences regarding blood transfusion in line with your "Personal Data Processed and Purposes of Processing" and to carry out the process for your sensitivities in the applied medical process. It will be processed in cases stipulated by law in accordance with the provision of express consent within the scope of paragraph 2 or in accordance with paragraph 3 of article 6.

Your special quality personal data regarding your health information is subject to the purpose of "Your Processed Personal Data and Purposes of Processing" and under the obligation of keeping the secret in accordance with the clause 2 of Article 6 of the Law No. It will be collected by authorized persons in order to protect public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing.

Your Personal Data is processed securely in accordance with the Law on the Protection of Personal Data in order to provide a more appropriate service in accordance with legal obligations or in company business and transactions.

In this context, the data in the table is collected as personal data;

Identity: It is the data group that contains information about the identity of the person. (Ex. Name, Surname, Date of Birth / Place of Birth, TR Identity Number, Gender, SSI Registration Number, Nationality, Marital Status)

Communication: It is the data group that can be used to reach the person. (Ex. Telephone Number, Settlement Address, E-Mail Address)

Physical Space Security: This data category refers to data types such as employee entry and exit registration information, Camera recordings. (Ex. Camera Image Recordings)

Transaction Security: This data category refers to data types such as IP address information, website login and exit information, Password and password information. (Ex. Security Keys, Log Records, IP Addresses)

Finance: It is the data group that contains the financial information of the person. (Ex. Bank Account Number, IBAN Information)

Audio-Visual Records: This is the data group that contains the visual and audio data of the individual. (Ex. Photo, Sound Recording, Camera Recording)

Health Information: It is the data group that contains the health information of the person. (E.g. Blood Type, Medical History, Check-Up Result, Consultation Report, Diet Form, Surgery Information, Applied Treatment Method, Disease Type, Medications Used, Eye Retinal Shots, Medical Values, Analysis Results).

Biometric/Genetic Data: It is the data group that contains the biometric/genetic data of the person (eg Fingerprint, Palm Print, Genetic Information).

Travel Data: It is the data group that contains information about the person's travels (eg Flight Information, Boarding Card, Tour Route, Mile Card Number, Accommodation Data).

In addition to the information above, when you want your preferences regarding blood transfusion to be determined;

Religious Knowledge: It is the data group that contains the data of the person's belief (For example, Religious Knowledge)

In addition to the information above, when you want to benefit from the discount in accordance with the agreement between our contracted associations, unions and non-governmental organizations and our hospital;

Union Information: It is the data group that contains the union information of the person. (Ex. Union Membership Information)

Association Membership Information: It is the data group that contains the member and association information of the person. (For example, Member Association Information).
